How we handle information submitted by hospice agencies

This notice describes how Find Hospices, operated by National Hospice Directory LLC ("Find Hospices," "we," "our"), handles information submitted by hospice agencies.

1. What We Collect From Hospice Agencies

When a hospice agency creates or manages a paid account, we collect:

  • Contact information for authorized account users
  • Provider-submitted business information (e.g., descriptions, additional services offered, branding assets)
  • Login credentials managed by our secure authentication provider
  • Payment method details processed through our PCI-compliant payment processor (we do not store full card numbers)

We do not collect any form of Protected Health Information (PHI).

2. What We Do NOT Collect

Find Hospices does not collect:

  • PHI
  • Patient names, diagnoses, or clinical details
  • Medical records or internal operational documentation
  • Any patient-identifiable data

Any PHI submitted in error is permanently deleted.

Integrity of Third-Party Data

Find Hospices does not alter Medicare Quality Ratings, CAHPS Survey results, Google Reviews, or any other externally sourced metric. These appear exactly as published by their original sources.

3. Security Practices

We use industry-standard safeguards:

  • HTTPS encryption
  • Secure authentication
  • Encrypted token storage
  • Strict role-based access
  • Continuous monitoring and infrastructure protection

4. Payments

Payments are processed by PCI-compliant third-party payment processors.

Find Hospices never stores full credit card numbers.

5. Sharing of Information

We only share necessary and relevant data with trusted service providers who assist in operating the platform (hosting, analytics, support, payment processing).

We do not sell provider information.

6. Your Choices

You may update or delete your paid account at any time.

A Basic profile using publicly available data may still appear in the directory after account deletion to help families access accurate hospice options.
