I am writing this review to document my experience with Transitions LifeCare during a recent transition of hospice services. When our services were terminated, we were provided with a specific reason for discharge at the time of the event. However, upon reviewing the transfer of care to a secondary provider, it was discovered that a completely different—and contradictory—reason had been documented in the official reports provided to the VA. ​This lack of transparency and the discrepancy in reporting created a chaotic and unnecessarily distressing transition for my elderly father, who relies on a stable and clear care plan. A hospice provider’s primary responsibility is to ensure that a patient’s health records and discharge status are communicated accurately and consistently, especially when transitioning care to a new provider. The failure to provide a unified, factual account of the discharge reason suggests a significant breakdown in administrative oversight and communication standards. ​I am sharing this to warn other families: if you choose Transitions LifeCare, ensure that every communication, reason for discharge, and medical assessment is provided to you in writing immediately. The inconsistency we experienced caused significant confusion and could have had serious implications for our patient’s care.

I cannot say enough good things about Josh and all of the staff at Transitions LifeCare. They jumped immediately when we needed it most. They were professional and quick and caring.
